Neothelid are great leviathans which can grow from the same tadpole-like larva as Mind flayers if the larva are left unattended.
Ezradaur, a mind flayer, told the party that a neothelid has arisen in the Underdark which implies a dark fate has befallen the underdark mind flayer city.
